Saskia Middeldorp: Key Takeaways from the 2026 ACC/AHA Guideline on Acute PE
Saskia Middeldorp, Head and Professor of Internal Medicine at Radboudumc, shared a post on LinkedIn about a recent article she and her colleagues co-authored, adding:
“There is a lot of excitement on the 2026 ACC/AHA guideline on acute pulmonary embolism and we applaud the authors.
Also, we were given the chance to write a commentary.
These are our take aways:
- The AHA/ACC acute PE clinical categories is appealing for interdisciplinary discussions, but it also risks becoming too complex for clinical use.
- Several components incorporated into the new risk group definitions have been validated as predictors of adverse outcomes, but have not been rigorously tested as part of a classification system meant to guide clinical decisions.
- The acute PE guideline adopts a liberal stance toward catheter-directed thrombolysis and mechanical thrombectomy, extending consideration of these modalities to a broad spectrum of patients, including some at comparatively low risk of adverse outcomes
- At least 10 randomized controlled trials are underway comparing the clinical efficacy and safety of novel treatment modalities to the current standard of care, and their results are expected over the next few years.”
