“At the ESC Congress, we will discuss the potential role of Factor XI/XIa inhibition in preventing non-cardioembolic stroke.
Why should cardiologists be interested?
Non-cardioembolic stroke is not a single disease.
It includes large-artery atherosclerosis, small-vessel disease, aortic and other vascular sources—and often coexists with coronary and peripheral artery disease.
Despite antiplatelet therapy and intensive vascular risk-factor control, substantial residual ischemic risk remains.
Yet intensifying conventional antithrombotic therapy comes at the cost of more bleeding.
Factor XI inhibition proposes a different strategy: target pathological thrombosis while preserving physiological haemostasis as much as possible.
The central question is therefore not simply whether FXI inhibitors cause less bleeding, but whether they can deliver sufficient protection from ischemic stroke.
At ESC, we will explore:
- why Factor XI may be relevant beyond atrial fibrillation
- which non-cardioembolic stroke mechanisms could benefit
- what previous trials have taught us about efficacy and safety
- why molecules, targets, doses, and degrees of inhibition are not interchangeable
- how cardiologists and neurologists can jointly identify the right patients
The future may not be more anticoagulation—it may be more precise modulation of thrombosis.”
