“Myth-busting on perioperative autologous red blood cell salvage. Overview of the cell salvage process

  1. myth: cell-salvaged blood is of poor quality/inferior to allogeneic RBCs: if properly collected/processed, quality of cell salvaged RBCs exceeds allogeneic blood; here some considerations on morphology, oxygen unloading, lifespan, hemolysis, immunological effects
  2. myth: cell-salvaged blood increases bleeding: residual heparin typically have limited impact… but large-volume acute blood loss still requires factors replacement!
  3. myth: cell-salvaged blood is not safe in many surgeries: oncologic, obstetric, contaminated surgeries are NOT absolute contraindications… balance/mitigated risks!
  4. myth: cell-salvaged blood is not cost-effective: generally it is, particularly if employed selectively for high-risk cases
