Wolfgang Miesbach: VOYAGER1 Highlights the Potential of Oral Inno8 at ISTH 2026

Wolfgang Miesbach, Professor of Medicine at Frankfurt University Hospital, shared on LinkedIn:

Inno8 – stepping toward oral haemophilia A prophylaxis.

Late‑breaking VOYAGER1 data at ISTH 2026 showed that IV and oral Inno8, a small FVIIIa‑mimetic molecule, was well tolerated in healthy adult men, with dose‑proportional PK, systemic exposure, and clear dose‑dependent effects on thrombin generation and aPTT.

No serious or severe AEs, no anti‑Inno8 antibodies, and no signals of off‑target diffusion to organs such as the brain were reported.

Inno8 is formulated with SNAC and niacinamide to enhance gastric absorption and protect the molecule from degradation, specifically targeting uptake in the stomach rather than the intestine.

All doses in VOYAGER1 were given in the fasted state (≥ 6 h), and while dose‑escalation was performed, exact dose levels were not disclosed; food effects and optimal real‑world administration therefore remain important open questions.

With its low molecular weight and distinct mode of action, Inno8 differs from other FVIIIa‑mimetic concepts, so direct comparisons of affinity and mechanism are not possible at this stage.

If confirmed in haemophilia A trials, this approach could mark a significant step toward truly injection‑free prophylaxis.”
