Baptiste Thibault: My First Publication as First Author on Ionized Calcium Monitoring Before Citrate-CRRT

Baptiste Thibault, Anesthesiologist and Critical Care Physician at Hospices Civils de Lyon (HCL), shared a post on LinkedIn about a recent article he and his colleagues co-authored, published in BMC Anesthesiology, adding:

“Proud to share my first publication as first author, published in BMC Anesthesiology.

Citrate anticoagulation is now the first-line strategy for continuous renal replacement therapy in the ICU. Yet no guideline says whether ionized calcium should be checked before we start. So we looked at what actually happens at the bedside.

In 200 critically ill patients treated with citrate-CRRT at our centre, we found that:

  • Ionized calcium was not measured before initiation in nearly 4 out of 10 patients
  • Among those measured, 40% were already hypocalcemic
  • Reassuringly, calcium levels returned to normal within the first 12 hours in most patients – including those starting hypocalcemic

A simple question we face every day — and still no guideline to answer it.
