Christoph B. Olivier: DOACs Gain Ground in Post-MI Left Ventricular Thrombus Management
Christoph B. Olivier, Head of the Clinical Trials Unit, UHZ Freiburg, shared a post on LinkedIn about a recent article he and his colleagues co-authored, published in Clinical Research in Cardiology, adding:
“Happy to share that our paper has been published in Clinical Research in Cardiology!
In this study, we analyzed a large cohort of 19,964 patients admitted with acute MI between 2008 and 2022 at the University Heart Center Freiburg – Bad Krozingen to evaluate LVT incidence, evolving prescription trends, and long-term outcomes.
We found that:
- Incidence in a high-volume cardiology center was low at 0.83 percent, reflecting the benefits of rapid modern coronary reperfusion.
- Discharge prescriptions shifted significantly over 15 years, with DOAC adoption rising to 75 percent in recent years.
- Complete thrombus resolution was achieved in 79 percent of follow-up cases, with no significant difference between DOACs and VKAs.
- Clinical outcomes-including bleeding events, systemic embolism, and all-cause mortality- were comparable between DOAC and VKA strategies.
- Initial thrombus size served as the main independent predictor of complete LVT resolution.
These findings support the expanding role of DOACs as an effective and safe alternative to VKAs in post-MI LVT management, while highlighting the crucial role of early diagnostic imaging.
