Marc Carrier: Important New Data From the API-CAT Trial on Recurrent VTE During Extended Apixaban Therapy

Marc Carrier, Professor at Ottawa Hospital Research Institute, shared on LinkedIn about a recent article by Isabelle Mahé et al, published in Circulation, adding:

”Exciting results presented by Prof Isabelle Mahé at the International Society on Thrombosis and Haemostasis (ISTH) 2026 Congress in Paris, with simultaneous publication in Circulation.

Key findings from this pre-specified ancillary analysis of the API-CAT trial include:

  • Recurrent VTE during extended apixaban therapy was uncommon, occurring in only 2.5% of 1,766 patients over 12 months.
  • The risk of recurrence differed by cancer type, with the highest incidence observed in gastrointestinal cancers (excluding colorectal).
  • The index VTE strongly predicted the pattern of recurrence. Patients presenting initially with PE were most likely to recur with PE, while those with isolated DVT recurred exclusively with DVT.
  • Despite its low frequency, recurrent VTE carried a very poor short-term prognosis, with 50% mortality occurring within a median of just 24 days following recurrence.”
