Sarah Moharem Elgamal: Cardiovascular Imaging Beyond Anatomy Toward Earlier Risk Prediction

Sarah Moharem Elgamal, Honorary Clinical Senior Lecturer at University of Liverpool, EACVI Education Awards Task Force, Communications Committee Member, EACVI Chair of Web and Communication, EACVI Board Member, Deputy Chair of the Cardiac CT Sub-Committee of EACVI Certification and Accreditation Committee at European Society of Cardiology, Editorial Board at European Heart Journal – Imaging Methods and Practice, Adult Congenital Heart Disease and Imaging Cardiology Consultant at Liverpool Heart and Chest Hospital, Emerging Editorial Consultant Journal of American College of Cardiology: Cardiovascular Imaging, shared a post on X:

“Three new studies, simultaneously published in the European Heart Journal – Cardiovascular Imaging alongside ESC Congress 2026, highlight how cardiovascular imaging is moving beyond anatomy toward haemodynamics, molecular biology and earlier risk prediction.

  • An MRI-based model offers a promising, non-invasive approach to estimating right atrial pressure—an important marker of right-sided filling pressure and systemic congestion.
  • Hybrid (¹⁸F)GP1 PET/MR provides direct in-vivo evidence linking platelet activation and microvascular thrombosis with microvascular obstruction after STEMI. Notably, platelet activity extended beyond abnormalities visible on cardiac MR, suggesting a potentially targetable biological pathway.
  • Longitudinal change in the left atrioventricular coupling index independently predicted incident HFpEF in the MESA cohort. An increase of 1.6% or higher per year identified participants with more than twice the adjusted risk of HFpEF.

Together, these studies show the growing potential of advanced imaging to measure pressure non-invasively, reveal active disease mechanisms and identify subclinical remodelling before heart failure develops.”
