The HELP-PCI trial studied 999 STEMI patients undergoing PPCI to compare the timing of UFH administration—at first medical contact (FMC) vs in the Cath Lab.
Patients who received UFH at FMC had significantly better initial coronary blood flow (TFG-3: 23.6% vs 17.6%; OR 1.44; P = .02).
No differences were seen in 12-month MACCE, reperfusion success, or major bleeding.
Early UFH may improve pre-PPCI perfusion without added risk.
