”Case Study Spotlight: Post-Thrombotic Iliac Intervention
I recently shared a fascinating case inside the Ultrasound Unlocked Case Study Library that highlights why detailed venous imaging matters.
Patient: 51-year-old male
History: IV drug use, family history DVT, Factor V Leiden, prior motorcycle accident → left leg DVT → post-thrombotic syndrome
Symptoms: Thigh/calf tightness, bursting pain after 1–2 blocks, hyperpigmentation, limited relief from anticoagulation + compression
Key Findings:
- All distal veins compressible, no reflux
- External iliac vein occluded up to hypogastric vein
- Left common iliac compressed by artery
- Velocity ratio 5.3 (>2.5 cutoff)
Intervention: successful iliac stent placement with excellent patency post-op
Takeaway: Careful duplex and IVUS evaluation can uncover hidden proximal obstructions that mimic arterial claudication, opening the door for effective endovascular treatment.
