”Subacute Coronary Occlusions: Navigating the Gray Zone in CAD Care
In CV medicine, we often think of coronary artery disease as either acute (requiring urgent evascularization) or chronic (managed primarily with GDMT).
But there’s a critical group in between that doesn’t fit neatly into either category: subacute coronary occlusions (SCOs).
These lesions present unique diagnostic and therapeutic challenges.
Unlike acute coronary syndromes, they may not demand immediate intervention-but unlike stable chronic disease, delaying or misjudging treatment can lead to myocardial damage and worse outcomes.
This “gray zone” requires thoughtful, patient-centered decision-making that balances symptoms, ischemic burden, viability, and procedural risk.
A recent review takes a deep dive into this evolving space, outlining how SCOs differ from both acute and chronic occlusions and why a tailored approach is essential for optimal care.
