“The New AHA/ACC PE Guidelines Just Landed, and They Finally Match Real Clinical Practice
The AHA and ACC have overhauled how we classify acute pulmonary embolism, moving away from the old massive/submassive/low-risk labels and into a far more clinically useful A–E model. It’s the first time the guideline framework actually mirrors what we see at the bedside in managing PE patients.
What I appreciate most is the focus on physiology. RV strain, biomarkers, and respiratory compromise now carry real weight in determining risk stratification. The addition of the respiratory modifier is especially important, anyone who manages PE knows that hypoxia often tells the story before physiologic hypotension does.
The new categories also give us a clear home for the patients who aren’t in shock yet but are headed there, which has always been a gray zone. This alone will improve communication across PERT teams and help with faster, more consistent decision making.
I believe it is meaningful step forward for the entire PE community, and a welcome update for those of us working in this space every day.”
