Yet they keep happening; not because we lack the knowledge, but because we fail to implement what we already know.
We need a new framework: the awareness that prevention is our responsibility, the precision tools to identify who is at risk, and the systems to make risk stratification mandatory — not optional.
Prevention shouldn’t depend on whether someone remembers to order it.”
