Niyas Khalid: A Rare Case of Venous Ischemic Pancreatitis Redefining Diagnosis

Niyas Khalid, Head of Department of Inpatient Care at Burjeel Hospital, shared on LinkedIn about a recent article he and his colleagues co-authored, adding:

”A patient presented with severe abdominal pain. But the pancreatic enzymes were low.

That contradiction changed everything.

We recently published a case:
Venous Ischemic Pancreatitis With Reduced Pancreatic Enzymes Secondary to Extensive Splanchnic Venous Thrombosis.

To the best of our knowledge, this represents one of the very few reported cases globally, and likely the – First reported case from the UAE, of this rare vascular phenotype of pancreatic injury.

What made this case different was not just its rarity, but the need to step away from conventional diagnostic pathways. Instead of excluding pancreatitis based on enzyme levels, deeper evaluation of discordant findings led to identification of a vascular cause, allowing timely anticoagulation and preventing serious complications.

This case reinforced an important lesson:
Clinical breakthroughs often begin with a simple question – ‘Why doesn’t this make sense?’
