Augustina Isioma Ikusemoro: Transfusion Should Be Based on Clinical Need, Not Just Numbers

“Transfuse the patient – not the number

A laboratory value alone should never be the reason a patient receives blood.

Blood transfusion is a powerful, life-saving therapy – but it is not risk – free, and it is not routine care.

Every unit transfused must answer one critical question:

Does this patient truly need it right now?

Clinical transfusion decisions should always consider:

  • Symptoms of inadequate oxygen delivery
  • Ongoing or active bleeding
  • Cardiovascular risk status
  • Acute clinical conditions (ACS, stroke, major surgery)
  • Physiological reserve and comorbidities
  • Hemoglobin thresholds in stable/hospitalized patients ( less than seven grams per deciliter)

Because unnecessary transfusions expose patients to real risks:

  • Transfusion reactions
  • Trali
  • Taco
  • Immunomodulation
  • Iron overload
  • Residual infection risk
  • Loss of scarce blood resources for those in urgent need

Good transfusion practice is not about correcting numbers.

It is about protecting patients.

Safe transfusion starts with:

  • The right patient
  • The right indication
  • The right timing

Better outcomes happen when transfusion decisions are clinically justified, evidence-guided, and patient-centered.
