Wolfgang Miesbach: Platelet Variation and How Do They Move Over Time?
Wolfgang Miesbach, Professor of Medicine at Frankfurt University Hospital, shared a post on LinkedIn:
”Platelet variation and how do they move over time?
That may be the main diagnostic criterion for ITP – not simply how low the platelet count is at a single time point.
At the recent EHA‑SWG Scientific Meeting on Bleeding and Platelet Disorders, John Semple gave a fantastic presentation highlighting the Platelet Variability Index (PVI):
ITP cases showed highly variable platelet curves with repeated peaks and deep troughs over weeks.
Non‑ITP thrombocytopenia (splenomegaly, MDS, cirrhosis) showed flat, almost straight platelet lines with little day‑to‑day or week‑to‑week change.
Mechanistically, this striking variability in ITP may reflect a dynamic interplay between autoantibody‑mediated platelet destruction, fluctuating megakaryocyte activity and TPO-receptor signalling, creating an unstable system in which small triggers can cause big swings.
We do not fully understand this yet, but it opens an exciting window into ITP biology and suggests that platelet trajectories over time may be as informative as the absolute platelet count itself.”
