Mukaram Rana: Anticoagulation-Free VV-ECMO for Trauma-Induced Pulmonary Haemorrhage in Fontan Physiology

Mukaram Rana, Resident in Cardiology at University Hospital Frankfurt am Main, shared on LinkedIn about a recent article he and his colleagues co-authored, published in European Heart Journal  Case Reports, adding:

”We are pleased to announce that our case report has been published in European Heart Journal – Case Reports

We describe a rare and challenging case of trauma-induced diffuse pulmonary haemorrhage in a patient with Fontan physiology, successfully managed with anticoagulation-free VV-ECMO support.

A 17-year-old patient with Fontan physiology developed progressive respiratory failure following blunt chest trauma.

Ongoing pulmonary bleeding precluded systemic anticoagulation during ECMO therapy.

A heparin-free VV-ECMO strategy with targeted haemostatic therapy enabled recovery while maintaining circuit patency.

The patient was successfully weaned from ECMO and discharged without residual organ dysfunction.

This case highlights the importance of personalized multidisciplinary care in high-volume MCS expert centres.”
