This session will examine what ‘precision’ in DOAC reversal looks like in real-world practice, including not only the choice of reversal strategy, such as andexanet alfa, idarucizumab, and 4-factor prothrombin complex concentrate (4F-PCC/PCC), but also administration logistics, timing, dosing, patient selection, governance, and the potential role of urgent DOAC drug levels.

The discussion will draw on insights from the HaemSTAR trainee-led network and the JTH paper, ‘A UK-wide analysis of 2265 patients receiving a reversal agent for direct oral anticoagulant-associated bleeding.’
