Abciximab is a monoclonal antibody antiplatelet agent used intravenously in high-risk patients undergoing PCI.
Mechanism of action
- Fab fragment of immunoglobulin
- Blocks glycoprotein IIb/IIIa receptors on platelets
- Inhibits platelet aggregation
- Helps prevent thrombus formation
Indication
- Used as an adjunct with aspirin and heparin
- High-risk patients undergoing percutaneous coronary intervention (PCI)
- Effects may last 24–48 hours after infusion
Dosage
- Initial loading: 250 mcg/kg IV bolus
- Infusion before angioplasty: 125 ng/kg/min, max 10 mcg/min
- Infusion after angioplasty: continue as per protocol, commonly up to 12 hours
Side effects
- Bleeding
- Hypotension
- Nausea
- Bradycardia
- Rare thrombocytopenia
Nursing responsibilities
- Monitor for bleeding from IV site, gums, urine, stool, and puncture sites
- Check platelet count and coagulation profile as ordered
- Monitor BP, HR, and ECG
- Avoid unnecessary IM injections
- Apply firm pressure after sheath removal
- Educate patient to report bleeding, dizziness, or black stools
