Johann Morelle: Complement Inhibitors Are Reshaping Modern Nephrology
Johann Morelle, Professor of Nephrology, Head of the Division of Nephrology at University Hospitals Namur (CHU UCL Namur), a post on LinkedIn about a recent article he and his colleagues co-authored, published in Nephrology Dialysis Transplantation:
“Complement inhibitors are rapidly transforming the management of kidney diseases – from complement-mediated thrombotic microangiopathies (TMA/aHUS) to glomerular diseases such as C3 glomerulopathy (C3G), primary immunoglobulin-associated MPGN (Ig-MPGN) and IgA nephropathy (IgAN).
In our new review published in Nephrology Dialysis Transplantation (NDT), the journal of the European Renal Association (ERA), we discuss the mechanistic basis of infection risk under complement inhibition (eculizumab, ravulizumab, iptacopan, pegcetacoplan) and propose a pragmatic framework for infection-risk mitigation in nephrology practice.
Key take-home messages:
- Infection risk under complement inhibition is target-dependent
- Prevention strategies should integrate vaccination, antimicrobial prophylaxis, patient education, and rapid-access pathways for febrile illness
- CKD, dialysis, transplantation, and concomitant immunosuppression further modulate infectious vulnerability.
Grateful to Mathilde Berghmans, Francois Seghers, Safak Mirioglu, and Andreas Kronbichler for the outstanding collaboration!”
