Marc Carrier: EPCAT III Trial at ISTH 2026 Supports Aspirin Alone for VTE Prevention After Arthroplasty

Marc Carrier, Professor at Ottawa Hospital Research Institute, shared on LinkedIn about a recent article by Sudeep Shivakumar et al, published in NEJM, adding:

Exciting news from the International Society on Thrombosis and Haemostasis (ISTH) 2026 Congress in Paris, where the EPCAT III trial was presented as a Late-Breaking Abstract with simultaneous publication in the NEJM Group.

This landmark Canadian-led RCT demonstrated that aspirin alone, initiated immediately after total hip or total knee arthroplasty, was noninferior to an initial 5-day course of rivaroxaban followed by aspirin for the prevention of symptomatic VTE, without a clinically meaningful difference in bleeding.

These are truly practice-changing results that have the potential to simplify postoperative thromboprophylaxis for millions of patients undergoing hip and knee arthroplasty worldwide while maintaining excellent efficacy and safety.”
