Shadi Tabibian: Low FVIII Does Not Always Mean Hemophilia A
Shadi Tabibian, Deputy of research at Blood Diseases Research Centers in Iran University of Medical Sciences, shared a post on LinkedIn about a recent article by Emmanuel J. Favaloro et al., published in Seminars in Thrombosis and Hemostasis, adding:
“Low FVIII does not always mean hemophilia A.
One important diagnostic pitfall is von Willebrand disease type 2N (VWD 2N), in which FVIII:C is disproportionately low while VWF studies may be normal or only mildly reduced.
In these cases, a low FVIII:C and VWF:Ag ratio (less than 0.7) should raise suspicion for type 2N VWD rather than automatically labeling the patient as hemophilia A.

When FVIII appears discordantly low, VWF:FVIIIB assay (or molecular testing) is essential for confirmation.
Correct distinction matters, because treatment approach differs between VWD type 2N and hemophilia A.”
