“Paraneoplastic Syndromes : Tumor with or without Mediator
Endocrine
1. SIADH with hyponatremia : small cell lung carcinoma : ADH
2. Cushing syndrome : small cell lung carcinoma : ectopic ACTH
3. Hypercalcemia without bone metastasis : squamous cell carcinoma of lung, renal cell, breast, HTLV1 adult T cell leukemia : PTHrP
4. Hypocalcemia : osteoblastic metastases (prostate) : calcium consumption
5. Hypoglycemia : hepatocellular carcinoma, fibrosarcoma, mesothelioma : IGF2
6. Polycythemia : renal cell carcinoma, hepatocellular carcinoma, cerebellar hemangioblastoma, uterine leiomyoma : erythropoietin
7. Carcinoid syndrome (flushing, diarrhea, right sided valve disease) : carcinoid with liver metastases or bronchial carcinoid : serotonin
Neurologic
8. Lambert Eaton myasthenic syndrome : small cell lung carcinoma : anti P/Q type voltage gated calcium channel antibody
9. Myasthenia gravis : thymoma : anti acetylcholine receptor antibody
10. Subacute cerebellar degeneration : ovarian, breast, small cell lung : anti Yo, anti Hu, anti Tr
11. Limbic encephalitis : small cell lung : anti Hu
12. Opsoclonus myoclonus (dancing eyes, dancing feet) : neuroblastoma in children, small cell lung in adults
13. Anti NMDA receptor encephalitis : ovarian teratoma
Hematologic
14. Migratory thrombophlebitis (Trousseau sign) : pancreatic, lung, gastric adenocarcinoma : mucin and tissue factor
15. Nonbacterial thrombotic (marantic) endocarditis : mucinous adenocarcinoma
16. Disseminated intravascular coagulation : acute promyelocytic leukemia, adenocarcinoma
17. Eosinophilia : Hodgkin lymphoma, mycosis fungoides : IL5
Dermatologic
18. Acanthosis nigricans : gastric adenocarcinoma : TGF alpha
19. Seborrheic keratoses erupting suddenly (Leser Trelat sign) : gastric and other GI adenocarcinoma
20. Dermatomyositis (heliotrope rash, Gottron papules) : ovarian, lung, gastric
21. Sweet syndrome (acute febrile neutrophilic dermatosis) : acute myeloid leukemia
22. Pemphigus (paraneoplastic) : non Hodgkin lymphoma, CLL, Castleman disease
23. Necrolytic migratory erythema : glucagonoma
Musculosceletal and renal
24. Hypertrophic osteoarthropathy with clubbing : lung adenocarcinoma (not small cell)
25. Membranous nephropathy : solid tumors, especially lung and colon
26. Minimal change disease : Hodgkin lymphoma
27. Amyloidosis (AL type) : multiple myeloma, plasma cell dyscrasia
Paraneoplastic means the tumor is speaking through a molecule. Name the molecule and the tumor follows automatically.
