Sequential Efgartigimod and Eltrombopag Achieve Durable Response in Refractory ITP – Platelets Journal

“Recently published in Platelets!
‘Durable platelet response and immature platelet indices following sequential efgartigimod and eltrombopag therapy in a patient with refractory ITP’ by Fumiya Wada, et al. (2026).
This case report follows a 53-year-old woman with refractory immune thrombocytopenia (ITP) who had cycled through numerous therapies without lasting benefit.
Efgartigimod, a neonatal Fc receptor (FcRn) antagonist, produced a partial response and when supplemented with the TPO-RA eltrombopag, the combination contributed to a sustained response.
Remarkably, after stopping efgartigimod at 22 weeks, remission continued for a further 24 weeks on eltrombopag alone.
The authors also tracked immature platelet indices throughout, observing a reciprocal shift, with IPF percent falling while AIPF rose, mirroring recovering platelet counts (see Figure 2, below).
This suggests that combining IgG depletion with enhanced platelet production may act synergistically in ITP, and these indices might serve as practical biomarkers for monitoring treatment response.”
