When to Escalate Treatment in Sickle Cell Crisis – ISBT

Sickle Cell Crisis: When to Escalate Therapy – Part 2

In part 1, we looked at the clinical red flags that can signal a complicated or organ-threatening sickling crisis. In part 2, we explore when red cell exchange (RCE) should be considered and where PLEX may fit in severe, refractory disease.

Key messages:

  • Know when urgent RCE may be indicated.
  • Reduce HbS while avoiding hyperviscosity.
  • PLEX is not routine first-line therapy.
  • Escalate early and involve a multidisciplinary team when severe organ dysfunction persists.

The goal: interrupt sickling, reverse organ injury and prevent irreversible damage.
