“A septic preterm infant’s platelet count climbs past 1,000,000.

In an adult, that number puts you on alert for arterial thrombosis. In this infant, nothing clots.

Interpreting that count through an adult framework leads you toward the wrong concern.

The number is real, the sepsis is real, but the risk it signals in a neonate isn’t the one you were trained to expect from an adult platelet count that high.

Same septic trigger.

Same alarming number.

Something about how this platelet answers when it’s triggered is not what you’d expect, and that gap is what keeps a count like that one from turning into a clot.
