” ‘5 CBC mistakes residents make’ 5/5
CBC Mistake N5: Pancytopenia equals leukemia.
Pancytopenia is a pattern, not a diagnosis.
Think broadly:
- Drugs/toxins
- Nutritional deficiency
- Viral infections
- Hypersplenism
- Aplastic processes
- MDS Acute leukemia
- Marrow infiltration
Then look at the reticulocyte count and peripheral smear.
Low reticulocytes – production problem until proven otherwise.
Abnormal cells on smear – stop and investigate further.
The CBC is the beginning of the hematology work-up – not the end.”
