Mohamed Magdy: The Hematological Consequences of Portal Hypertension in Banti Syndrome
Mohamed Magdy, Physician at Ministry of Health Oman, shared a post on LinkedIn:
“Banti Syndrome
Banti syndrome is a condition characterized by splenomegaly (enlarged spleen) associated with portal hypertension and hypersplenism, leading to excessive destruction of blood cells.
Pathophysiology: Portal hypertension – Congestion of blood in the spleen – Splenomegaly (enlarged spleen) – Hypersplenism
Increased destruction/sequestration of blood cells:
- Red blood cells – Anemia
- White blood cells – Leukopenia
- Platelets – Thrombocytopenia
Causes:
- Liver cirrhosis
- Portal vein obstruction/thrombosis
- Other disorders affecting portal blood flow
Clinical Features:
- Massive splenomegaly
- Fatigue and pallor (due to anemia)
- Increased infections (due to leukopenia)
- Bleeding tendency or bruising (due to thrombocytopenia)
- Features of portal hypertension (e.g., varices)
Key Exam Point: Banti syndrome equals to Portal hypertension plus Splenomegaly plus Hypersplenism
A patient with an enlarged spleen and pancytopenia (low RBCs, WBCs, and platelets) should raise suspicion for Banti syndrome/hypersplenism.”
