Elad Asher։ STORM-PE Shows Functional Benefits of Thrombectomy
Elad Asher, Professor of Medicine at Shaare Zedek Medical Center, shared a post on LinkedIn about a recent article by Rachel P. Rosovsky et al, published in JACC: Cardiovascular Interventions, adding:
”Reperfusion in intermediate-high risk pulmonary embolism has always had an evidence problem: the endpoints that moved were anatomic, and the endpoints patients care about were never tested in a randomized trial.
The STORM-PE 90-day report in JACC Cardiovascular Interventions is the first attempt to close that gap.
One hundred patients at 22 international sites were randomized 1:1 to computer-assisted vacuum thrombectomy with anticoagulation, or anticoagulation alone. The trial’s primary endpoint, reported separately in Circulation in 2025, was the change in RV/LV ratio at 48 hours.
This paper reports functional, quality-of-life and safety outcomes through 90 days.
The functional signal is real and it is large. Six-minute walk distance was 478.7 plus or minus 159.5 metres with thrombectomy against 368.0 plus or minus 127.8 metres, with a p-value of 0.003. NYHA class I at 90 days was 97.4 percent (37 out of 38) against 75.6 percent (34 out of 45), with a p-value of 0.005. Dyspnea and quality-of-life scores were comparable, and there was no difference in all-cause mortality, PE-related mortality or symptomatic recurrence.
Now the part that decides how much weight this carries. The walk test was completed by 31 of 47 patients in the thrombectomy arm and 34 of 53 in the anticoagulation arm. Roughly a third of each arm is absent from the headline endpoint, and in a walk test, non-completion is not random: the patients who cannot manage the corridor are the ones whose distance you most need. Add nequal to 100, secondary endpoints, and means with standard deviations but no confidence intervals, and this is a signal worth pursuing rather than a settled answer.
It is still the first randomized evidence that catheter reperfusion changes how these patients function months later, and that is not nothing.
What would convince you – a larger trial, or complete follow-up on this one?”
