“Antiplatelet therapy in 2026: duration is becoming a dynamic decision
Key takeaway: contemporary antiplatelet therapy is moving from fixed regimens toward individualized intensity, duration and de-escalation based on evolving thrombotic and bleeding risk.
- Elective PCI for CCD: typically 6 months DAPT.
- ACS without high bleeding risk: more or equal to 12 months remains the default.
- Selected patients may transition after 1–3 months to P2Y12 monotherapy.
- With long-term anticoagulation, triple therapy should generally be minimized.
Comment: shorter DAPT is not inherently better.
Many trials enrolled selected, relatively lower-thrombotic-risk populations.
The more important concept is serial reassessment, because bleeding and ischemic risk evolve differently over time.
