Diego Escarraman Martinez: MATTERs Study – When a Cohort Gave Us Major Clues about TXA in Trauma

Diego Escarraman Martinez, Anesthesiologist, shared a post on X about a recent article by Jonathan J. Morrison et al., published in JAMA Surgery, adding:

“MATTERs Study (2012): when a cohort gave us major clues about tranexamic acid in trauma.

In medicine, not all relevant evidence comes from randomized clinical trials.

In 2012, Morrison and colleagues published the MATTERs study in Archives of Surgery, a retrospective cohort of 896 patients with combat trauma in Afghanistan, of whom 293 received tranexamic acid (TXA).

What did they find?

Hospital mortality was lower in those who received TXA:

  • 17.4% vs. 23.9% in the general cohort (p = 0.03).
  • 14.4% vs. 28.1% in patients with massive transfusion (p = 0.004).

In this latter group, TXA was associated with a higher probability of survival (OR 7.23; 95% CI 3.02–17.32), even though the treated patients had more severe injuries.

Being a retrospective cohort, its results are subject to selection bias and residual confounding. Therefore, a statistical association does not prove causality.

However, MATTERs provided important evidence on the benefit of TXA in severe hemorrhagic trauma, complementing the later findings of the randomized clinical trial CRASH-2 published in 2010.

Remember that in Mexico, Defsa laboratories offers high-quality Tranexamic Acid.”
