Denis Oduor: Whole Blood and Component Therapy in Clinical Practice

Denis Oduor, Nairobi County Blood Services Coordinator at Nairobi City County, shared a post on LinkedIn:

”Whole Blood and Component Therapy: Let Clinical Need Guide the Choice

Whole blood remains an important transfusion option in selected clinical situations, particularly massive haemorrhage, where rapid replacement of circulating blood may be required. Component therapy, however, remains essential for targeted treatment and for maximising the use of each donated unit.

The World Health Organization’s 2025 postpartum haemorrhage guidelines emphasise clinical assessment, clear transfusion protocols and the appropriate selection of blood products. They favour component therapy over whole blood alone, while allowing consideration of whole blood where components are unavailable and appropriate compatibility safeguards are met.

Emerging evidence in civilian trauma also supports further evaluation of whole-blood transfusion in carefully selected patients. Nevertheless, evidence from trauma must not automatically be extrapolated to obstetric haemorrhage.

For Nairobi and other resource-constrained settings, our priority should be to strengthen reliable access to safe, screened and appropriately stored blood products, maintain functional component-processing capacity, and establish clear emergency transfusion protocols.

The objective is not whole blood versus component therapy. It is the right product, for the right patient, at the right time.

Blood should not wait for the patient.”
