Perioperative Care in Cancer

Cancer patients on blood thinners face a uniquely difficult moment when surgery can’t wait.

We pooled data from two rigorous randomized VTE trials (ADAM VTE with EVE) to ask:

What actually happens when we interrupt anticoagulation for a procedure in cancer-associated thrombosis?

What we found – 179 patients, 290 procedures:

  • Bleeding risk was approximately 3 times higher with injectable LMWH (dalteparin) vs. oral apixaban
  • Zero recurrent blood clots at 30 days – in any group
  • Procedure ‘risk category’ alone didn’t predict who bled – the low and moderate risk group had the highest bleeding rate

The precision medicine takeaway:

Anticoagulant choice, cancer biology, and individual context all matter – and apixaban appears to offer a more manageable perioperative profile.

This is exactly the kind of individualized, data-driven decision-making that precision medicine promises — now applied to one of oncology’s most common and understudied clinical scenarios.”
