Eleftheria Hatzimichael: ASH 2026 Guidelines for Primary ITP What Is Changing

Eleftheria Hatzimichael, Special Secretary of the Board of Directors at Hellenic Society of Hematology, shared a post on LinkedIn:

“ASH 2026 Guidelines for Primary ITP — what is changing?

The new ASH focused update brings an important shift in the management of adults with primary ITP: moving beyond corticosteroids alone and beyond platelet count as the main therapeutic goal.

For initial therapy, rituximab plus corticosteroids or a thrombopoietic agent plus corticosteroids are now suggested over corticosteroids alone.

After failure of first-line corticosteroids, thrombopoietic agents receive a strong recommendation, while rituximab remains an important alternative.

Perhaps the most important message, however, is broader: treatment should focus on outcomes that matter to patients  bleeding, fatigue, quality of life, treatment burden and sustained response off therapy, not simply achieving a higher platelet count.
