Hussnain Javaid: New EHA Guidelines for HbSC Disease Management

Hussnain Javaid, Consultant Haematologist at NHS UK, Founder of ClinHaem, shared a post on LinkedIn:

”HbSC disease: EHA’s first dedicated guideline is here.

Key updates:

For symptomatic HbSC disease, the guideline suggests choosing between phlebotomy & hydroxyurea, based on the patient’s complications, steady-state Hb, suitability and preference.

  • Phlebotomy may suit patients with higher Hb—particularly greater than 105 g/L—or the possibility of hyperviscosity-related complications, retinopathy,. Suggested target: Hb 95–105 g/L, with Hb, haematocrit and ferritin monitoring.
  • Hydroxyurea may suit patients with frequent VOC, recurrent ACS or other severe disease complications.

If response is inadequate, consider combining treatments; exchange transfusion may be considered for severe or progressive complications after specialist review.

Other key recommendations: annual retinal examination from age 10; annual kidney screening from age 10; VTE prophylaxis for admitted patients over 12 unless contraindicated; and transfusion targets based on COMBINED HbS plus HbC.

These are consensus-based recommendations, and HbSC-specific evidence remains limited.”
