“The Vascular Dimension of Long COVID
One striking observation in Long COVID is how rarely endothelial dysfunction is considered, despite fatigue, cognitive dysfunction, exertional intolerance, and dysautonomia suggesting impaired microvascular regulation.
The endothelium regulates vascular tone, platelet activation, coagulation, inflammation, and tissue perfusion. Dysfunction can impair microcirculatory flow without a conventional thrombotic event. ‘Microclots‘ may be one manifestation – not the diagnosis itself.
This matters therapeutically. In the appropriate phenotype, treatment should address endothelial inflammation, platelet or coagulation activation, vascular tone, and autonomic dysfunction – not simply ‘treat microclots.'”
