When Postpartum Headache Is More Than Migraine – National Blood Clot Alliance

“Postpartum CVST: a case worth remembering

A patient presented two weeks postpartum with a headache initially characterized as consistent with migraine – later found to be cerebral venous sinus thrombosis (CVST) with concurrent deep vein thrombosis, and near-complete occlusion visualized on imaging.

The postpartum period carries a significantly increased risk of venous thromboembolism.

Risk is highest immediately following childbirth – reaching approximately 100-fold above baseline during the first postpartum week – and remains elevated for up to 12 weeks after delivery.

CVST remains a rare but important diagnosis to consider in this population.

Its presentation – including persistent or progressively worsening headache, sometimes accompanied by neurological symptoms – can overlap with more common causes of postpartum headache, including migraine, tension-type headache and post-dural puncture headache.

For clinicians evaluating postpartum patients with atypical, persistent or refractory headache, recognizing red flags and considering appropriate imaging can be critical to timely diagnosis and improved outcomes.”
