Samwel Mikaye: From Virchow’s Triad to Prevention of PE in Patients with DVT

“Deep vein thrombosis (DVT) is a condition where a blood clot forms in the deep veins, most commonly in the lower limb (calf or thigh).

It is dangerous because the clot can travel to the lungs and cause a pulmonary embolism.

Causes (Virchow’s triad)

DVT is mainly caused by three factors:

1. Venous stasis (slow blood flow)

  • Long bed rest
  • Prolonged travel (sitting for long hours
  • Paralysis

2. Endothelial injury (vessel damage)

  • Surgery
  • Trauma
  • IV catheters

3. Hypercoagulability (increased clotting tendency)

  • Pregnancy
  • Cancer
  • Oral contraceptives
  • Genetic clotting disorders

Symptoms

  • Swelling of one leg (unilateral)
  • Pain or tenderness in calf
  • Warmth in affected leg
  • Redness or discoloration
  • Prominent veins

Dangerous complication

  • Pulmonary embolism (clot moves to lungs)
  • Sudden chest pain
  • Shortness of breath
  • Collapse (life-threatening)

Diagnosis

  • Doppler ultrasound (first line)
  • D-dimer test (screening)
  • CT/MR venography (if needed)

Treatment

1. Anticoagulation (main treatment)

  • Heparin (initial)
  • Warfarin or DOACs (long-term)

2. Supportive care

  • Leg elevation
  • Pain relief
  • Compression stockings

3. Severe cases

  • Thrombolysis (clot dissolving drugs)
  • Inferior vena cava (IVC) filter if anticoagulants cannot be used

Prevention

  • Early mobilization after surgery
  • Hydration
  • Compression stockings
  • Avoid long immobility
