Shivir Moosai: Warm AIHA in a Young Woman
Shivir Moosai, Hematology Specialist at North West Regional Health Authority, shared a post on LinkedIn:
“Warm Autoimmune Haemolytic Anaemia: Classic Blood Film Features
A 22-year-old woman presented with marked lethargy, widespread joint pains and anaemia, three months after an episode of pneumonia.
Bloods
- Hb 81 g/L
- MCV 105 fL
- Reticulocytes 9 percent
- Normal WBC
- Normal platelets
- Low C3 and C4
- Marked polyclonal rise in immunoglobulins
Blood film
Spherocytes and polychromasia, consistent with active haemolysis. Few NEBC’s.
Direct antiglobulin test
IgG strongly positive (3 plus).
Diagnosis
Warm autoimmune haemolytic anaemia.
Given her age, joint symptoms, low complement and polyclonal hypergammaglobulinaemia, an underlying autoimmune disorder, particularly SLE, is an important consideration.”
