Wilfried Dinh: Antiplatelet Therapy in 2026: Duration is Becoming a Dynamic Decision

Wilfried Dinh, Senior Clinical Program Leader at Boehringer Ingelheim, shared a post on LinkedIn:

“Antiplatelet therapy in 2026: duration is becoming a dynamic decision

Key takeaway: contemporary antiplatelet therapy is moving from fixed regimens toward individualized intensity, duration and de-escalation based on evolving thrombotic and bleeding risk.

  • Elective PCI for CCD: typically 6 months DAPT.
  • ACS without high bleeding risk: more or equal to 12 months remains the default.
  • Selected patients may transition after 1–3 months to P2Y12 monotherapy.
  • With long-term anticoagulation, triple therapy should generally be minimized.

Comment: shorter DAPT is not inherently better.

Many trials enrolled selected, relatively lower-thrombotic-risk populations.

The more important concept is serial reassessment, because bleeding and ischemic risk evolve differently over time.
