Oliver Weingärtner: Deprescribing of Lipid Lowering Drug after MI is Associated with Cardiovascular Events

Oliver Weingärtner, Professor of Medicine at Friedrich-Schiller University Jena, shared a post on LinkedIn:

“Study Alert: Deprescribing of lipid lowering drug after MI is associated with cardiovascular events!

What happens when we transition post MI patients from specialized care back to general practice?

The ‘Jena auf Ziel’ (JaZ) prospective cohort study followed 85 STEMI patients over 24 months to find out.

While 100% of these patients initially reached their strict target LDL-C of <55 mg/dL (<1.4 mmol/L) through early, combination therapy, their paths diverged at the 12-month mark.

Patients chose between staying with the University Hospital Jena’s specialized lipid clinic (62%) or transitioning to standard care with their local General Practitioner (38%).

The 24-Month Results:

  • Average LDL-C: Clinic patients maintained significantly lower levels than the GP group (1.2 mmol/L vs. 2.1 mmol/L).
  • Time on Target: Clinic patients spent much more time within their target range (82.4% vs. 62.4%).
  • The Driving Factor: Treatment de-escalation and deprescribing was associated with reduced LDL-C target attainment.

The Ultimate Clinical Impact (MACE):

This isn’t just about numbers on a lab report; it’s about survival. Patients who experienced a Major Adverse Cardiac Event (MACE) during the follow-up (n=10) had a shorter time on LDL-C targets (58.1% vs. 79.1%) and higher LDL-C levels at 24 months (2.04 mmol/L vs. 1.27 mmol/L).

Conclusion:

The study found an association between sustained LDL-C target achievement and lower cardiovascular event rates

Healthcare leaders and Cardiologists:

How can we better educate and collaborate in lipid networks to prevent premature deprescribing and protect high-risk cardiac patients after MI?”
