“TTP vs ITP vs DIC

TTP

ADAMTS13 is low – vWF multimers is high – platelet microthrombi

  • Platelets is low and hemolytic anemia
  • Schistocytes and is highLDH
  • PT/PTT NORMAL

ITP

Autoantibodies – platelet destruction is high

  • Isolated thrombocytopenia
  • PT/PTT NORMAL

DIC

Systemic coagulation activation – platelets plus clotting factors consumed

  • Platelets is low and PT/PTT is high
  • D dimer is high and fibrinogen is low
  • Schistocytes may be present

USMLE key:

  • TTP – ADAMTS13 is low and normal PT/PTT ITP
  • Isolated thrombocytopenia and normal PT/PTT
  • DIC – consumption coagulopathy and prolonged is high PT/PTT

Treatment:

  • TTP – Plasma exchange
  • ITP – Steroids and maybe IVIG
  • DIC – Treat the underlying cause”
