“TTP vs ITP vs DIC
TTP
ADAMTS13 is low – vWF multimers is high – platelet microthrombi
- Platelets is low and hemolytic anemia
- Schistocytes and is highLDH
- PT/PTT NORMAL
ITP
Autoantibodies – platelet destruction is high
- Isolated thrombocytopenia
- PT/PTT NORMAL
DIC
Systemic coagulation activation – platelets plus clotting factors consumed
- Platelets is low and PT/PTT is high
- D dimer is high and fibrinogen is low
- Schistocytes may be present
USMLE key:
- TTP – ADAMTS13 is low and normal PT/PTT ITP
- Isolated thrombocytopenia and normal PT/PTT
- DIC – consumption coagulopathy and prolonged is high PT/PTT
Treatment:
- TTP – Plasma exchange
- ITP – Steroids and maybe IVIG
- DIC – Treat the underlying cause”
