Ali Rafat: Targeting the Inflammatory Engine of Myeloproliferative Neoplasms

Ali Rafat, Assistant Professor of Anatomical Sciences at Kashan University of Medical Sciences, shared a post on LinkedIn about a recent article he and his colleagues co-authored, published in Cancers, adding:

“I am delighted to share that our article, ‘Inflammatory and Immune Microenvironment in Myeloproliferative Neoplasms: Pathogenic Mechanisms and Therapeutic Opportunities,’ has been published in the journal of Cancers.

In this review, we explore the complex interplay between chronic inflammation, immune dysregulation, and clonal evolution in Philadelphia-negative myeloproliferative neoplasms, including polycythemia vera, essential thrombocythemia, and primary myelofibrosis.

We discuss how driver mutations involving JAK2, CALR, and MPL contribute not only to abnormal hematopoietic signaling but also to the establishment of a persistent inflammatory microenvironment.

This inflammatory and immune landscape plays an important role in disease progression, bone marrow fibrosis, thrombosis, splenomegaly, and leukemic transformation.

The review also highlights alterations in both innate and adaptive immunity, including impaired T- and B-cell function, abnormal natural killer cell maturation, and the accumulation of immunosuppressive myeloid-derived suppressor cells.

Furthermore, we discuss the limitations of current therapeutic strategies and emerging opportunities to target the inflammatory and immune mechanisms underlying MPN progression.

We hope that this work will contribute to a better understanding of the dynamic relationship between malignant hematopoietic clones and their microenvironment and support the development of more effective, disease-modifying therapeutic strategies for patients with MPNs.
