Unexplained hemolysis, disproportionate anemia or atypical thrombosis in a myeloid neoplasm should prompt consideration of PNH testing.

Use high-sensitivity FLAER-based flow cytometry with lineage-specific clone quantification.

PNH can coexist with MPN or CML. Distinguish clinically significant disease from an incidental clone.

Integrate hemolysis, symptoms, thrombosis and clonal context. Clone detection alone does not justify complement inhibition.

Treat the patient, monitor the clone, and target complement when clinically indicated.”
