DOACs and Drug Interactions in Transplant Patients – JTH
Journal of Thrombosis and Haemostasis shared a post on LinkedIn about a recent article by Andreas Verstraete et al., adding:
“Direct oral anticoagulant use and drug-drug interactions in transplant recipients receiving calcineurin inhibitors.
Cyclosporine and tacrolimus inhibit the CYP3A4 and P-glycoprotein pathways that clear DOACs, raising concern about elevated drug levels.
Among 216 transplant recipients with 360 DOAC measurements, 11% were above-range, more often with rivaroxaban (20%) and apixaban (18%) than edoxaban (4%).
Higher levels tracked with strong CYP3A4 or P-glycoprotein inhibitors, lower weight, reduced kidney function, and cyclosporine use.”
