Maria Cherska: Are We Overlooking the Spleen in Cardiometabolic Research?

Maria Cherska, The Head of Cardiology Department at The State Institution V.P. Komisarenko Institute of Endocrinology and Metabolism, shared a post on LinkedIn about a recent article by Giovanni Tarantino and Vincenzo Citro, published in Lipids in Health and Desease, adding:

When discussing lipid metabolism, we often focus on the liver and adipose tissue.

A review by Giovanni Tarantino and Vincenzo Citro, published online as an article in press in Lipids in Health and Disease, brings the spleen into the conversation.

‘The spleen as a metabolic rheostat: orchestrating systemic lipid flux and interorgan crosstalk’ explores how lipid handling, immune activity and communication between organs may intersect.

Three themes caught my attention:

  • Cholesterol trafficking: macrophage pathways involved in lipoprotein processing, cholesterol efflux and HDL remodeling.
  • Platelet-derived extracellular vesicles: potential links between platelet biology, inflammatory signaling and lipid homeostasis, illustrated in the accompanying figure.
  • Spleen–liver communication: mechanisms that may help connect local immune responses with systemic metabolic changes.

For those working in lipidology, obesity and cardiometabolic medicine, this review offers a valuable perspective on the connections between metabolism and immunity.

The authors also make an essential distinction: much of the evidence comes from experimental models or related cellular systems. The integrated spleen-centered model remains a working hypothesis, with important questions still requiring validation in humans.
