Nico Gagelmann: iMTSS to Dissect the Drivers of Post-Transplant Outcomes
Nico Gagelmann, Physician at University Medical Center Hamburg-Eppendorf, shared a post on LinkedIn about a recent article he and his colleagues co-authored, published in Transplantation and Cellular Therapy, adding:
“What does overall survival actually tell us about why a patient is at risk?
In the largest genomically annotated myelofibrosis transplant cohort to date (n=1,550), we developed and validated iMTSS to dissect the drivers of post-transplant outcomes.
The aim is to move beyond estimating survival toward understanding what drives an individual patient’s risk. To improve understanding and future study designs.
Key findings:
- Relapse was primarily driven by disease biology: particularly TP53 allelic state, alongside RAS pathway mutations, ASXL1, driver genotype, blasts, and blood counts.
- Non-relapse mortality was driven by a distinct set of patient and transplant factors, including performance status, donor type, age, and portal vein thrombosis.
- These two risk dimensions were nearly independent.
- Integrating both dimensions, iMTSS outperformed established disease- and transplant-specific prognostic systems for overall survival.”
