Stroke is the leading cause of new-onset seizures and epilepsy in older adults and is linked to higher morbidity and mortality.
Post-stroke seizures are classified as early or late, depending on whether they occur within or after 7 days of the stroke, with a single late seizure sufficient to diagnose post-stroke epilepsy.
This review outlines current strategies for diagnosing and managing post-stroke seizures, highlighting levetiracetam and lamotrigine as potentially preferred options for reducing acute seizure recurrence despite limited evidence.
It also emphasizes the lack of data on several other agents and suggests that future research, particularly into biomarkers of epileptogenesis, could enable more practical and informative clinical trials.
