Nishant K Mishra։ When Epilepsy Meets Anticoagulation

Nishant K Mishra, Stroke Director at U.S. Department of Veterans Affairs, shared on LinkedIn about a recent article he and his colleagues co-authored, published in JAMA Neurology, adding:

”Pleased to share our new study in JAMA Neurology: ‘Safety of Antiseizure Medications During Direct Oral Anticoagulant Therapy in Epilepsy.’

In this international real-world study of adults with epilepsy receiving direct oral anticoagulants (DOACs), antiseizure medication choice was associated with meaningful differences in thromboembolic, bleeding, and mortality outcomes.

Notably, compared with lamotrigine/lacosamide, levetiracetam was associated with a higher thromboembolic risk, as were strong enzyme-inducing antiseizure medications.

These findings raise an important and very practical question: When treating epilepsy in a patient who requires anticoagulation, could antiseizure medication selection itself be a modifiable contributor to vascular outcomes?
