Piotr Czempik: ROTEM in Sepsis-Related Coagulopathy
Piotr Czempik, Chair of Department of Anesthesiology and Intensive Care at Medical University of Silesia, shared a post on LinkedIn about his recent article, published in Journal of Clinical Medicine, adding:
“Sepsis continues to challenge clinicians with rapidly evolving disturbances in coagulation.
I’m excited to share the new study exploring how ROTEM helps differentiate functional hemostatic patterns in SIC and overt DIC.
In a cohort of 167 ICU patients:
SIC and DIC both demonstrate prolonged CT/CFT, reduced early clot amplitudes, and suppressed fibrinolysis.
Platelet contribution to clot strength was the only ROTEM‑derived discriminator between SIC and overt DIC.
Importantly, no ROTEM parameter predicted short‑term mortality once adjusted for age and sex.
These findings reinforce the role of viscoelastic testing in bedside assessment while reminding us that sepsis‑related coagulopathy is multifactorial and not easily captured by single metrics.”
